STEH Street Pop Agency
The STEH Street Pop agency was created in Clarity to enable street outreach workers and other social services workers to create a record in Clarity for persons living on the street. This record can help document the person's history of homelessness and more easily connect them to services.
Street Pop (Certified)
The Street Pop (Certified) program in Clarity is available for outreach workers who may engage a person or family experiencing street homelessness and are not eligible for their outreach program. For example, a PATH worker may engage with a homeless youth who is not eligible for PATH outreach. Or a RHY outreach worker may engage a homeless veteran who is not eligible for a RHY outreach project.
Street Pop (Certified) doesn't require the detailed data collection other outreach programs require. However, Outreach workers are held to the Cincinnati/Hamilton Cty CoC Street Outreach Program Intake Policies and Procedures (coc-comprehensive-policy-manual.pdf (strategiestoendhomelessness.org, pg. 143) when entering data for Street Pop (Certified). Street Outreach staff must visit where the individual/family reports sleeping before entering them into Street Pop (Certified).
Entering the person's information into Street Pop (Certified) allows for:
- The client's homelessness to be documented during that initial engagement.
- The client's Verification of Homelessness Report to reflect the client was homeless at the time they were engaged rather than waiting until another more appropriate outreach program engages the client.
- The additional data entered into Street Pop can be used to validate chronic homelessness.
- Outreach workers can refer clients they engage to the most appropriate outreach program.
Street Pop (Non-Certified)
Street Pop (Non-Certified) is a supportive services program intended to collect data for persons at imminent risk of street homelessness or persons whose street homelessness has not yet been verified. The process for entering data is the same as the Street Pop (Certified) program. Persons enrolled in the Street Pop (Non-Certified) program will not be reflected on the person's Verification of Homelessness Report. This enrollment can be used to connect the person to the most appropriate outreach program.
Access to STEH Street Pop in Clarity
Outreach workers, support services staff, or their supervisors can request access to the STEH Street Pop agency. The following must be completed before access is granted:
- Completion of required HMIS training.
- Approval from the agency HMIS contact (CHO PPP) and the appropriate STEH staff.
- Signed HMIS User agreement (required upon login to Clarity).
Users must also participate in additional training to maintain access to Clarity as required by the STEH HMIS team. HMISsupport@end-homelessness.org must be notified within one working day if a user's access should be terminated.
Street Pop Data Entry
- Follow the instructions in the Step-by-Step Client Entry article to search for an existing client record or create a new record.
- Follow the instructions in the Program Enrollments article to create Street Pop project enrollment. Note: the Street Pop enrollment has been streamlined to include the minimal data entry requirements.
- Enter an Encounter Service to document contact with the client.
- Click on the program-level "Provide Services" tab.
- Expand the "Encounter" service.
- Enter the date of the encounter.
- Click on the "Add Location" button to include the location of the encounter. Users can use the Geolocation feature or manually enter the location. Click "Add."
- Enter notes about the encounter.
- Click "Submit."
- Street Pop (Certified) only: Enter a "Current Living Situation" assessment. This is required for the client's homelessness to be reflected in the Verification of Homelessness Report. Review the Current Living Situation article for additional information.
- Click on the program-level "Assessments" tab.
- Enter the required data.
- Click "Save."
Refer to the Appropriate Outreach Project
Street Pop has been set up so any user can refer the client to the appropriate outreach project. Street Pop users should never refer clients to a shelter or housing project. Clients seeking a referral to shelter or housing should contact the CAP line: 513-381-7233 phone, 513-970-1515 text.
1. Click on the "Referrals" tab from the client's profile screen.
3. Click on the 'By Program Category" button.
4. Choose Street Outreach and click the "Search" button.
5. Expand the appropriate outreach program and click "Make Referral."
6. Enter any relevant notes and click "Send Referral."
Referral Notifications and Outreach Project Enrollment
Users set up to receive referral notifications will receive an email notifying them that a referral has been made.
Click on 'This client' to review the client record. The referral details can be accessed from the client profile or the client's "History" tab.
Assign a case manager for the client.
Click on 'this link' to review the referral details.
Make sure the 'Program Placement a result of Referral provided...' toggle is on when enrolling the client into the appropriate outreach program. This will close the referral.
