How to: ESNKY Navigation Center

The ESNKY Navigation Center requires program enrollments in Clarity HMIS. This article will walk through the following process:

  1. Search for a Guest or Add a New Guest Record
  2. Create a New Program Enrollment
  3. Document Contacts and Progress in the Provide Services Tab
  4. Document Current Living Situation and Referrals in the Assessments Tab
  5. View Client Guest Contact and Public Alerts
  6. Exiting a Guest for the Navigation Center 


Search for a Guest or Add a New Guest Record 

  1. Search for the guest. 
  2. Click the edit icon to access the guest’s record if found in the search or click the “ADD CLIENT” button and complete the profile data if the guest is not found when doing a search.

     
  3. IMPORTANT! Update the Guest’s Profile
    Check that the data on the guest’s profile is up to date. Enter Prior Address and Contact Information fields if missing.
  4. Check for Services on Suspension (SOS)  
    If the guest has a current SOS (services on suspension), a “Public Alert” banner will display on the guest’s profile screen. Click “Review Notes” on the banner to review the SoS.

Create a Program Enrollment

  1. Click on the PROGRAMS tab. 

  2. Scroll down to the PROGRAMS: AVAILABLE section and click on the arrow next to ESNKY Emergency Shelter. 
     
  3. Click the ENROLL button and complete the enrollment. 

  4. Complete the enrollment and hit “SAVE & CLOSE.”

5. Document Contacts and Progress in the Provide Services Tab

5.1  Click on the HISTORY or PROGRAMS tab from the client record. Click on the edit icon next to the ESNKY Navigation Center program. 

 

5.2 Document contacts and assistance obtaining personal documentation on the Provide Services tab. 

a. Click on the program-level Provide Services tab. 

b. Select Case Management to enter contact notes. Click “Submit” to save. 

c.  Select Assistance Obtaining Personal Identification to enter assistance provided. Choose the appropriate sub-item, enter notes if needed, and click “Submit” to save. 

d. Click on the program-level History tab to view services history and notes.  

 

 

6. Document Current Living Situations and Referrals in the ESNKY Navigation Center Assessments Tab

6.1  Click on the HISTORY or PROGRAMS tab from the client record. Click on the edit icon next to the ESNKY Navigation Center program. 

 

6.2 Click on the program-level Assessment tab. Then click the START button next to the assessment to be completed.  


6.3 Enter the guest’s current living situation and document referrals. Click Save. 

6.4 View previously completed assessments from the program-level Assessment tab under Assessment History. 

View Client Guest Contacts and Public Alerts in the Notes Tab 

  1. There are two categories available: 
    1. Client Notes - these are where notes from other programs will be added.
    2. Public Alerts

**All notes must be set to PRIVATE.


Exiting a Guest from the Navigation Center

  1. From the guest’s record, click on the Client Level HISTORY or PROGRAMS tab. 
  2. Scroll over the current ESNKY Navigation Center enrollment and click the edit icon. 
  3. Click on the EXIT button on the left side of the Program Level navigation. button.
  4. Complete the exit and click the “SAVE & CLOSE” button.  For more detailed instructions, please see this help article.
