The table below describes goal topics collected for the Case Plan V3 assessment. Use the Assessment Date to enter the date the Case Plan was created or updated.


After saving the initial assessment, users should create a new Case Plan to track progress. The information from the previous plan will populate on the screen. Users should update the data to be current as subsequent assessments are completed. 


When any goals are set to 'achieved/maintaining,' a goal achieved date will be displayed so users can document the exact date the goal was determined to have been achieved. Users can document additional goals and save the updated case plan.


Data Collection Fields

Toggle yes/on to any of the goal categories listed below to enter the following fields:


  1. Shelter goal target date for completion: (enter date)
  2. Shelter goal status (select one):
    1. in progress
    2. achieved/maintaining 
      1. Goal achieved date (enter date goal achieved)
  3. no longer applicable

Shelter goal notes: enter specific details about the goal and progress


"Other" goals

Knowing no goal will meet every need, using "Other" goal options (e.g. Other Income/Skill Goal) will allow users to enter a goal related to that goal type.


Subcategories

When toggled yes/on, some categories will load additional sub-categories. Toggle yes/on any subcategory to access the above data collection fields.
