Overview

The Arizona Self-Sufficiency Matrix (ASSM) measures various domains of life and is beneficial to those who may have multiple factors contributing to their homelessness or need for shelter, services, or substance abuse recovery. The ASSM assists adults in need of improving their life skills.

This assessment is available in many programs within Clarity and may be required of clients (or only the Head of Household when working with a household) at the project start and anytime there has been a change to their homelessness situation.

The tool helps:

  • Case managers have a better understanding of their clients, their needs, and their life skills.
  • Case managers make informed decisions regarding case planning and interventions.
  • Improve outcomes for participants.
  • Evaluate the effectiveness of different programs.

Accessing the Assessment

To locate the ASSM, go to the Assessments tab in the program-level navigation. Select START next to "Arizona Self-Sufficiency Matrix (ASSM)" to begin.

Completing the Assessment

The assessment has various picklists, with each answer in a picklist having a corresponding point value. Immediately following the assessment, a score and a recommendation are made:
