Arizona Self-Sufficiency Matrix (ASSM)
The Arizona Self-Sufficiency Matrix (ASSM) measures various domains of life and is beneficial to those who may have multiple factors contributing to homelessness or need for shelter services, substance abuse, and services that assist adults in need of improving their life skills.
This assessment is available in many programs within Clarity and may be required of clients (or only the Head of Household when working with a household) at the project start and any time there has been a change. Please speak to your supervisor
The tool helps:
- Case manage to have a better understanding of their clients and their needs and/or life skills
- Case managers make informed decisions regarding case planning and interventions
- Improve outcomes for participants
- Evaluate the effectiveness of different programs
To locate the ASSM, go to "Assessments" in the program level navigation. Select "START" next to "Arizona Self-Sufficiency Matrix (ASSM)"
The assessment has various picklists. In the Assessment Date Field, enter the date the assessment was completed. Then, enter the option that best fits the client's current status or situation for each topic listed below.

Instead, please send the Clarity Unique Identifier found on the client's Clarity Profile page. You can use the messaging system in Clarity if you need to send sensitive personally identifying information.
