Annual Assessment


Purpose

The Annual Update Assessment must be completed within Clarity HMIS no more than 30 days before or after the anniversary of the head of household's Project Start Date.  Please note that this is actual days - not a calendar month in which days could vary.


Even if there have been no changes in status or information (disability type/status, income, benefits, or health insurance ) from the Project Enrollment or last Status Update Assessment - a new Annual Assessment is required on the anniversary of an individual's or household's Program Start Date.


Process

From an active client enrollment, go to "Assessments" in the program level navigation.  Select "START" next to "Annual Assessment."



Update any of the information that has changed since either the Program Enrollment or the last Status Update Assessment. 

  • Disabling Conditions and Barriers
  • Health and Wellness / Health Insurance
  • Employment
  • Monthly Income and Sources
  • Non-Cash Benefits

Add any additional comments to the text box:

Select "SAVE & CLOSE" (or "SAVE & NEXT" if completing the Status Update Assessment for additional household members.



There are some instances where information on the Enrollment screen should be updated/edited:

  • Error Corrections: for information that was incorrect or omitted at the time of enrollment
  • Housing Move-In Date: This information must be entered/updated on the enrollment screen
