Status Update Assessment


Purpose

The Status Update Assessment must be completed within Clarity HMIS whenever there is a change to an individual's disability type/status, income, benefits, or health insurance while the client is actively enrolled in a program.  This information is used to track changes over time. There is no limit on the amount of Status Update Assessments that can be entered at any point throughout an active Program enrollment.


Process

From an active client enrollment, go to "Assessments" in the program level navigation.  Select "START" next to "Status Update Assessment."

Update any of the information that has changed since either the Program Enrollment or the last Status Update Assessment. 

  • Disabling Conditions and Barriers
  • Health and Wellness / Health Insurance
  • Employment
  • Monthly Income and Sources
  • Non-Cash Benefits

Add any additional comments to the text box:

Select "SAVE & CLOSE" (or "SAVE & NEXT" if completing the Status Update Assessment for additional household members.



There are some instances where information on the Enrollment screen should be updated/edited:

  • Error Corrections: for information that was incorrect or omitted at the time of enrollment
  • Housing Move-In Date: This information must be entered/updated on the enrollment screen
