Effective immediately all Emergency Shelters funded by ESG are requested to collect COVID-19 vaccine-related information during current and future client enrollments. This policy is to help providers connect clients experiencing homelessness to available vaccines, better understand the reasons persons experiencing homelessness choose not to be vaccinated, and provide information about COVID-19 Vaccinations to help inform clients.
What you need to know
- Vaccine fields will be available for data collection in Clarity HMIS for ESG funded Emergency Shelters.
- Vaccine fields will be collected on enrollment and status update/annual assessment screens for clients ages 12 and older.
- Although the fields are required in Clarity, 'client refused' and 'client doesn't know' are acceptable answers to account for clients who wish to not share this information.
- This policy is specific to Emergency Shelters; however, any program interested in collecting this information can request the fields be added for other project types by contacting HMISsupport@end-homelessness.org.
