About the Report

The Assessment Details Report lists all the responses to the questions on the selected assessment. The responses for each client are included in the report. 


The report allows you to download a record of your clients’ assessment details in an Excel spreadsheet.  The report will download automatically as a CSV file (which opens in MS Excel) and will contain profile details (such as name, age, and DOB) as well as every answer to the assessment for every assessment completed within the reporting period. This can be helpful for reviewing rental data collected on the Rental Update assessment, or any other screen you can access from the Assessment tab in Clarity (except Status Updates, Annual Updates, or Current Living Situation). 



Note: This report requires an Assessment during the reporting period.  


Reading the Report


Each report is going to have the same basic information about the client: 

  • Unique ID
  • Staff Name
  • Program Name
  • Client Name
  • Last 4 of SSN
  • Age 
  • Race
  • Ethnicity
  • Gender

After that, each report dives into its own specific information. 


Locating and Running the Report

Click the Launcher and choose 'Reports'. Click the down arrow in 'Assessment Based Reports' to select [GNRL-210] Assessment Details Report. 


Click 'Run'. If you don't want to run the report immediately, click 'Schedule' to open a calendar function. Choose the date (up to a month ahead of time) you want the report to run. 


Choose the assessment results you want to download. Select one or multiple assessments. Hold the SHIFT button and left-click your mouse to select consecutive items; hold the CTRL button and left-click for separate items. 

Enter the report date range, and click 'Submit'.


Note: Unlike most other reports, this report only offers one output format: Excel.  This is because the number of columns can be quite large, and Excel is the best format for handling and viewing a large number of columns. The report is delivered as a zip file containing two Excel workbooks:


  • Assessment List
  • [The Name of the Assessment] 



Once submitted, the Report Queue Manager box pops up, telling you your report is being processed. 


When the report is complete, a second popup box will appear. Open either by clicking "Open" on the "Report is Ready" popup box or the number box at the top right of the screen.

A small box showing the report name appears on the bottom of your screen. Click the box to open the ZIP file. 

The ZIP file contains the names of all the assessments you chose. 

Click the assessment name to open it in Excel.
