Enrollment
To enroll a client in Shelter Diversion, bring up the client’s profile. Click on the “Programs” tab, then click the down arrow for “Shelter Diversion”.
Click the "Enroll" button on the bottom left of the screen.
Enrollment Fields
Housing Move-In Date
The Housing Move-In Date is the date the client will begin to occupy the new housing. Enter the date in the calendar.
Note: The Housing Move-In Date must be between the Project Start Date and Project Exit Date. It may be the same date as Project Start if the client moves into housing on the date they were accepted into the program.
There can be no more than one Housing Move-In Date per enrollment. Once a Housing Move-In Date has been recorded for an enrollment, it should not be removed from the client’s record, even if they subsequently move from that housing situation. Users must be able to edit data only to correct errors.
Type of Residence
Select the "Type of Residence" under "Living Situation"
Note: Adult members of the same household may have different prior living situations.
*Interim housing is not a type of housing but rather a housing situation for a client that meets the following criteria:
- Must have been chronically homeless at start in interim housing,
- Must have applied for permanent housing, accepted, and have a unit/voucher for permanent housing reserved for them,
- Must have been prevented from immediately accessing permanent housing unit or using a voucher in a permanent housing unit (e.g. apartment getting painted, old tenant moving out, has a voucher, but is looking for the unit, etc.) and
- Client and transitional housing project must have determined that transitional housing is an acceptable option until permanent housing unit is ready for occupancy.
Length of Stay in Prior Living Situation
Length of Stay in Prior Living Situation should reflect the length of time the client was residing in the living situation above. If the client moved around, but in the same type of situation, include the total time in that type of situation. If the client moved around from one situation to another, only include the time in the situation selected in the "Type of Residence" field above.
Disabling Conditions
Choosing "Yes" to any disabling conditions and barriers opens the "Long term/impairs independent living" field. Select "Yes" or "No." Note: If you choose "Yes" to any "Long term/impairs independent living" field, choose "Yes" in the "Disabling Condition" field. If "Long term/impairs independent living" is "Yes," and "Disabling Condition" is "No," the information reported may not be accurate.
